Final Forms and Information for Parents

Parents-- ALL PLAYERS must have an updated physical and must complete all forms on Final Forms before they tryout for the team.  These forms can be found below. Completed physical forms must be uploaded to Final Forms.

 

The Player Fee this year is $85.  The fee includes a team shirt, the use of a uniform for the season, and 2 passes for admittance to home games.  Parents should expect to have to pay to get into away games.
