If you are bringing a non-Central Magnet School date, you must submit the completed paperwork
along with a copy of your dates photo ID at the time of ticket purchase.
You will not be allowed to purchase tickets for an outside guest without proper documentation.
Paperwork for a date that attends a high school other than Central Magnet School is attached below.
Form must be turned in to ticket seller in order to purchase tickets for prom. Please also email the forms to Ms. Benjamin at firstname.lastname@example.org. Your date will not be allowed to enter prom if Ms. Benjamin does not have proper forms sent to her by email as well as uploaded to go fan.
