Assessment Remediation Plan (only for major tests!!!)

-utilize directed study or lunch periods to re-teach or discuss the material one-on-one as needed.

-re-take policy for major tests (unit/summative assessments)

-Original score = A then complete 1 of the following activities prior to taking a re-test

-Original score = B then complete 2 of the following activities prior to taking a re-test

-Original score = C then complete 3 of the following activities prior to taking a re-test

-Original score = D or below then complete 4 of the following activities prior to taking a re-test

-the re-test must be attempted within 1 week of the original test date (this may be adjusted as needed)

-you must let me know when this will be taken…5th period (lunches B-C-D) is the best option

Relearning Activity Options

-Correcting your test/quiz- redoing all missed problems with an explanation as to why the correct answer is correct.

-PLTW, Crash Course, Bozeman science, TED videos (only if there is one available on the standard or topic missed) and take notes as proof of viewing.

-Make flashcards/ or use Quizlet of important vocabulary/concepts…show flashcards complete as proof.

-Tutoring by an adult (must turn in a note from the adult that tutored you)

-Make a foldable/graphic organizer with test material (bring to me and show me as proof)

-(if available on topic) Additional handout on topic to be completed and correct!

-(if applicable) Colored Diagram/Drawing/Label of related body part to topic (for example if on the ear you would draw/color/label an ear)!
